What are the side effects of the alternative to estrogen, alendronate (Fosamax)?

Alendronate sodium () is one of a new group of drugs, biphosphonates, used in treatment ofosteoporosis and Paget’s disease of bone. The package insert issued by Merck & Co., Inc., in March, 1996, for Fosamax indicates that irritation of the upper gastrointestinal tract is a major concern. This is especially true for the esophagus, where inflammation, erosions and even ulcers may occur and lead to heartburn and painful swallowing. In order to enhance absorption, the drug (in tablet form) is taken with water after getting up in the morning and before eating. In fact, it is best to avoid food or any other medication for at least 30 minutes after taking this drug. In addition, one should not lie down during this period. A history of esophageal disease increases the risk of trouble as does the use of aspirin. Antacids may interfere with the drug’s absorption and should not be taken with alendronate.
Based on limited experience with alendronate during pregnancy or breast feeding, it is best to avoid use under those conditions. In addition, it should not be taken by people who have low blood levels of calcium.
As is true of use of any medication, some people may experience side effects that are rare or previously unrecognized. Report possible reactions to the physician who prescribed the drug or to the dispensing pharmacist.
